Appointments

Patients are seen in the office by appointment only.  For your first appointment we ask that you please arrive fifteen minutes early to complete the necessary paperwork.  To save time, please go to the Forms Download page to get the necessary forms for your appointment.  New patients must complete both the Registration form and the Privacy Consent form.  For established patients, these forms must be completed at least once per year, or more frequently if your information changes.  In addition to the forms, please bring your current insurance card, your copay (if applicable) and any necessary referrals. 

We have two locations, Bel Air and Havre de Grace, where patients may schedule an appointment.  All of our providers rotate between the two offices, so you may see your provider in either office.  You may call either office to schedule an appointment between the hours of 9 AM and 4:30 PM.  To schedule an appointment in Bel Air, please call 443-643-4300.  To schedule an appointment in Havre de Grace, please call 410-939-3121.

If you are late for an appointment it may be cancelled and rescheduled for another time, depending on the provider’s schedule.  If you are unable to keep a scheduled appointment, please call us as soon as possible.  We request a minimum of 24 hours notice. This courtesy will allow us to schedule another patient who is waiting for medical attention.  If you have an urgent issue, we will make every effort to insure that you are given an appointment with the first available provider.
